The NTA JEE Main self-declaration form 2022 is a part of the JEE Main hall ticket. It will be printed on the front page of the admit card of JEE Main 2022. Due to the ongoing COVID-19 pandemic, candidates must not only bring their JEE Main admit cards to the exam hall but also carry a duly filled and signed self-declaration form. A candidate who does not bring the signed self-declaration form to the exam hall is liable to be denied admission to the exam hall by the exam authorities. The necessity of the undertaking lies in the fact that candidates will have to declare if they have travelled in the last 14 days, if they have any ongoing symptoms of Covid-19, or if they have come in contact with any Covid-19 patients and details of their quarantine period if any.
